Description
Two identical forms with hand written responses. Both have photos attached. A third form has the same information, although no photo. Some of the information on the third form has been crossed out. Both are signed by Chin Youk, and the Deputy collector of Customs. Attached is a note from the Collector Of Int. Rev. District of Oregon saying that Chin Youk submitted an identical form on Jan 24, 1894 in Portland, Oregon.
